How do I submit my proof of Covid vaccine or qualified exemption?

Proof of vaccination (such as digital record or vaccination card) is to be provided to Stephanie Newman (snewman@assistanceleaguela.org) or Yvonne Bradshaw (ybradshaw@assistanceleaguela.org) in Human Resources via email. You may also mail in a copy or present your card in person. The League will not keep a copy of your vaccine status, and once it is documented you will not have to show proof again.
Should you qualify for an exemption under a strongly-held religious belief or due to a medical condition, YOUR EXEMPTION MUST BE VERIFIED WITH HUMAN RESOURCES IN ADVANCE, and you will be required to show proof of a negative COVID-19 test taken within the prior three (3) days to visit the Chapter House or attend a League event. If either of these exemptions applies to you, please contact Yvonne or Stephanie as soon as possible.
